Our dayroom is a place for our participants to enjoy activities, meals and socialization. Bingo, crafts, trivia, walking club, board games, worship services and planned events are scheduled throughout the month. We also have a “Memory Lounge” for individuals who may need more one-on-one care throughout the day.
Weekly fitness classes to maintain strength and function. Classes on topics such as: healthy cooking, mindfulness, bible study, and more. Quiet areas to rest and relax. Showers and, personal grooming and laundry areas. Nutrition support provided by our dietitians at the PACE Center.

Door through door transportation services to PACE Center and to specialty Medical Appointments. Drivers provide safety oversight for participants during loading and unloading.
Drivers are CPR /and first aid trained and must pass a drug screening and background check.

A primary care clinic with exam rooms and a triage area at PACE Center.
Primary Care services provided by our In-house primary care providers.
Nursing Care provided in PACE clinic. On-call services available 24 hours per day, 365 days per year.
Specialty services including dental, vision, audiology, podiatry as approved by PCP and IDT.
Lab work, X-rays, and Surgery Emergency care
Hospital care.
Prescription and over the counter medications provided at no copay.

Made of Primary Care Provider, Registered Nurse, Social Worker, Physical Therapist, Occupational Therapist, Recreational Therapists, Registered Dietitian, Home Care Coordinator, Transportation Representative, Day Center staff member and Center Director.
Coordinated care by the PACE Interdisciplinary Team provides a review of each participant’s care plan every six months or more often if needed to ensure participant needs are addressed.
IDT collectively acts as a case manager to each participant and approves all services.

Delicious, warm, and healthy meals are served on weekdays at the PACE center. Take-home meals are also available if needed.

Therapy gym with parallel bars, exercise bikes, weights, resistance machines to improve movement and safety at PACE Center.
Physical therapy services to develop, maintain, and restore maximum movement and functional ability provided by licensed therapists at the PACE center.
Occupational therapy service to assist individuals develop, recover and improve the skills needed for daily living at the PACE center.
Home visits and modifications to ensure a safe living environment Durable Medical Equipment such as canes, walkers, and wheelchairs.

Cleaning and preventative care by a Dental Hygienist who is a contracted provider in the community.
Dental work provided by a PACE dentist or contracted provider in the community.
Dentures based on assessed need.

Mental health services with contracted providers to enhance independence and growth of our participants.
Grief and support services.
Social workers and case managers assist with provision of additional resources as needed.
Spiritual services provided by PACE Chaplain.

Participant and family-centered care that is focused on improving quality of life and relieving suffering in the last years, months, weeks or days of life.
Care addresses physical, psychological, social and spiritual needs.
Care provided in your home or community setting by the PACE team.
Respectful of individual autonomy, self-determination, and personal wishes.
