About
The Targeted Case Management (TCM) program provides comprehensive services through a recovery-based model to adults and children who struggle with serious and persistent mental health challenges. TCM works to enhance participants’ mental wellness and community integration by providing access to needed resources that move them forward in their recovery and help them achieve their individual goals.
Eligibility
Participants must be enrolled in or eligible for the Maryland Medical Assistance Program, have a severe mental health disorder, and be at risk of (or need) continued community treatment to prevent:
- Inpatient psychiatric treatment
- Homelessness
- Incarceration
